PALLIATIVE CARE

Palliative Care provides supportive, consultative care for individuals living with a serious illness, with a focus on comfort, symptom management, and quality of life. The Pathways Palliative Care Program is designed to support patients who are continuing curative treatment while helping them manage the physical, emotional, and practical challenges of illness.

Our palliative care team works alongside a patient’s existing physicians to provide an added layer of support. In addition to symptom relief and care coordination, the Pathways program can help identify and connect patients with additional services and resources that may be beneficial throughout their care journey.

Eligibility for Palliative Care

Palliative care may be appropriate for individuals who:

  • Are living with a serious illness

  • Are continuing curative or life-prolonging treatment

  • Have a physician willing to collaborate with the palliative care team

Care Settings

Palliative care services may be provided in:

  • The home

  • Long-term care facilities

  • Hospitals

Our Care Team

The Pathways Palliative Care team may include:

  • Nurse Practitioners

  • Registered Nurses

  • Physicians

  • Medical Social Workers

  • Chaplains

Services are provided regardless of a patient’s ability to pay.
