The Community Alternatives Program (CAP) supports adults with disabilities by helping them remain safely in their own homes while maintaining choice, independence, and control over their care.

Palliative Care focuses on comfort, symptom relief, and support for individuals living with serious illness—at any stage and alongside curative treatment.

PHYSICIANS
Our Physicians play a vital role on the hospice care team, providing medical leadership, guidance, and compassionate oversight for each patient’s care. They work closely with nurses, caregivers, and families to manage pain and symptoms, ensure comfort, and support informed decision-making throughout the care journey. With a focus on dignity and quality of life, hospice physicians help create personalized care plans that honor each patient’s needs, values, and wishes.
Supportive Services provide emotional, spiritual, and practical support to patients and families throughout their care journey. Our team—including social workers, chaplains, volunteers, and bereavement specialists—offers guidance, resources, and compassionate care during illness and through the grieving process.
