Social Work services are a vital part of the hospice care team, offering guidance, advocacy, and emotional support for patients and families facing a life-limiting illness. Our Social Workers are compassionate, knowledgeable professionals who are committed to serving each individual with respect, dignity, and understanding.
Each patient and family is assigned a dedicated Social Worker who helps address the emotional, social, and practical challenges that often arise during hospice care. Their focus is to enhance quality of life by providing support, education, and connection to helpful resources.
Emotional Support
Social Workers provide individualized emotional support tailored to the needs and wishes of each patient and family. This may include supportive counseling or simply being present to listen, offer comfort, and provide companionship during difficult moments.
Advance Care Planning
Social Workers assist with completing advance directives, including naming a healthcare power of attorney and documenting wishes regarding medical treatment at the end of life.
Community Resources & Assistance
Your Social Worker can help connect you with community and financial resources, including:
Meals on Wheels and food assistance programs
Heating and utility assistance
CAP services
Transportation services
VA benefits
Long-term Care Medicaid
Nutritional Support
When illness or reduced appetite makes nutritional supplements necessary, Social Workers can help apply for funding to assist with the cost of items such as Boost or Ensure, which are not typically covered by Medicare.
Personal Safety
For patients who live alone or are without a caregiver around the clock, Social Workers can arrange for medical alert systems that allow 24-hour access to help. While not covered by Medicare, funding is available through Hospice of the Blue Ridge to support this essential service.
Home Safety & Independence
Social Workers may assist with accessing grant funding for minor home repairs or equipment that promotes safety and independence, such as grab bars, lift chairs, and wheelchair ramps.
Final Wishes & Legacy Support
We are honored to help patients fulfill meaningful final wishes—whether that’s a special trip, creating memories with loved ones, or preserving stories and messages for family members. Our Social Workers work with community foundations to help make these wishes possible whenever they can.
After care and Bereavement services
Our care does not end when a loved one passes—it continues with compassionate support for those left behind. Hospice aftercare and bereavement services are designed to walk alongside families through the grieving process, offering guidance, comfort, and a listening ear during one of life’s most difficult times. Through counseling, support groups, and personalized outreach, we help individuals and families find healing, honor their loved one’s memory, and navigate the path forward at their own pace.
