COMMUNITY ALTERNATIVE PROGRAM (CAP)

The Community Alternatives Program (CAP) helps eligible adults with disabilities remain safely in their own homes rather than entering a long-term care facility. CAP services are designed to provide the support individuals need to live independently while maintaining choice, dignity, and control over their daily lives and care decisions.

Through CAP, participants receive personalized services that support health, safety, and overall well-being while allowing them to remain in familiar surroundings. Services are tailored to meet individual needs and are coordinated to promote independence and quality of life.

CAP Program Options

CAP/DA (Disabled Adults)
CAP/DA serves adults age 18 and older who have limited or no informal support systems and who are at risk of placement in a long-term care facility.

CAP/Choice
CAP/Choice allows participants greater control over their services by enabling them to direct and manage their own care with additional support from a Care Advisor.

Eligibility Requirements

Individuals may be eligible for CAP services if they:

  • Are 18 years of age or older

  • Are eligible for Medicaid

  • Live in a private residence

  • Are at risk for nursing home placement

  • Have health and well-being needs that can be safely managed at home within Medicaid cost limits

Services May Include

CAP services are based on individual needs and may include:

  • Adult day health services

  • Home accessibility modifications and adaptations

  • Personal care assistance

  • Meal preparation and delivery

  • Institutional and non-institutional respite services

  • Personal emergency response systems

  • Medical equipment and supplies

  • Participant goods and services

  • Community transition services

  • Training, education, and consultation

  • Case management

  • Care Advisor services (CAP/Choice only).
